Three Eyes

Patient displays a glowing red third eye, sometimes with normal eyes visible beneath it.

Hollow Face

The patient has hollow eye sockets and a wide-open mouth with a hunched posture; linked to dangerous later events.

Wide Eyes and Creepy Smile

Unnaturally enlarged eyes paired with an unsettling grin.

Human Teeth

Realistic human teeth instead of normal animal dentition.

Mismatched Eyes and Sharp Teeth

One eye sits higher than the other, while sharp teeth are visible.

Different Eyes

The eyes in the photograph don't match the patient, missing pupils or added eyelids.

Different Ears

Ear shape, size, or animal type differs between the photo and the patient.

Incorrect Photo

The photograph displays a different animal or appearance entirely.

Unnatural Photo

Distorted facial expressions visible only in the photograph, such as blurred features or creepy smiles.

Static Photo

A thick layer of static covers the entire photograph.

Cursed Photo

Picking the photo up before it fully processes drains sanity.

Censored Eyes

A black bar covers the patient's eyes, with realistic eyes staring back through the camera feed.

Unnatural Body

The body stretches into impossible proportions that change with each camera view.

Staring at Cameras

The patient keeps their eyes locked on the CCTV camera wherever it moves.

Void Bodies

The patient's body appears completely black with no visible features.

Skinwalker

A monstrous shapeshifter that is only visible through CCTV footage.

Camera Twitching

Twitching movements visible only on the security feed, brief and easy to miss.

Bed Monster

Long black hands protrude from under the hospital beds.

Head Banger

A white cat-like creature repeatedly slams its head against the check-in window.

Ghost

A translucent figure with floating particles, strange sounds and self-opening doors.

Stalker

A tall black creature with long ears and oversized eyes that flickers the lights before revealing itself. Taking corners slowly and looking away as soon as it appears stops the attack.

Hollow Face (CCTV)

A separate sign from the appearance version of Hollow Face. This one only shows up when the patient is viewed through CCTV, not standing at the door.

Mismatched Ears or Eyes (CCTV)

Subtle, easy to miss if you only glance at the camera once. Worth a second look before admitting.

Shapeshifter

Also called the Mimic in some community guides. It enters looking like an ordinary patient and only reveals itself after getting close.

Tendril

Shows up mid surgery on certain patients rather than at check in.

Camera Figure

Only appears through broken security cameras, never in person.

Mass of Eyes

Looking straight at it drains sanity fast, several points every half second.

Hiders

Detaches from the wall and chases once it notices a player nearby.

Slimebert

Added in a later update. Roams during the Team Hazmat event around Shifts 11 to 12, and disappears once that event ends.
